Documentation from Healthcare Provider

Qualified professionals may include, but are not limited to, physicians, psychologists, audiologists or other licensed authorities as determined by the type of disability.
The documentation provided must include the diagnosis of the disability, the impact of the disability in an educational setting, the severity and longevity of the condition, and suggestions for appropriate learning strategies and accommodations.
